Requirements;

  1. Filled claim form
  2. Photo evidence
  3. Copy of Driver’s License
  4. Drivers comprehensive statement
  5. Police Abstract (accidental)

life & funeral claim

Requirements;

  1. Filled claim form
  2. Copy of the Deceased’s ID
  3. Copy of Beneficiary’s ID
  4. Certified copy of the burial permit
  5. Police Abstract (accidental)
  6. Medical summary (If death happened at the hospital)
  7. Chief’s acknowledgment letter (If death happened at home)
  8. Claimant’s bank details
