- Filled claim form
- Photo evidence
- Copy of Driver’s License
- Drivers comprehensive statement
- Police Abstract (accidental)
life & funeral claim
Requirements;
- Filled claim form
- Copy of the Deceased’s ID
- Copy of Beneficiary’s ID
- Certified copy of the burial permit
- Police Abstract (accidental)
- Medical summary (If death happened at the hospital)
- Chief’s acknowledgment letter (If death happened at home)
- Claimant’s bank details
